Attention Deficit Disorder Self Evaluation
The following Self-Screening Test will provide you with a means of determining whether or not you should proceed with a comprehensive evaluation. This questionnaire should not be used as the sole basis of a self-evaluation.
Adult Self-Evaluation
- Do you have difficulty sustaining attention or shift from one task to another?
- Do you have difficulty completing projects at work or at school?
- Does your spouse, partner, or roomate often complain that you are detached, out-of-touch or insensitive to the feelings of others?
- Are you impatient or do you become frustrated easily?
- Do you often feel as if you are hyperactive or can't slow down?
- Do you often interrupt or intrude on others?
- Do you often lose things or find that you are forgetful?
- Do you have difficulty managing your time or procrastinating?
If you have answered "Yes" to any two or more of the above, or to number 5 alone, it is recommended that you proceed and register to take our comprehensive evaluation.
Child Self Evaluation
- Does your child often exhibit signs of restless, overactive behavior?
- Does your child often show impulsive behavior (quick to act without thinking)?
- Do you have trouble getting your child down to sleep due to their overactivity or inability to relax?
- Does your child fail to finish things he/she starts or is easily distracted?
- Do you receive frequent complaints from teachers regarding inattention, trouble staying on task or overactivity?
- Does your child often appear lost in thought, daydreaming of out of touch?
- Does your child often disturb or have trouble interacting with other kids?
- Does your child have difficulty following instructions?
If you have answered "Yes" to any two or more of the above, or to number 5 alone, it is recommended that you proceed with a further comprehensive evaluation for your child.
